We all need a name, right? This should include a first and last name, and if you want to give a middle name that's cool, too. Nicknames and whatnot can also go here.

Age
Again. No explanation needed.
Appearance
I want a fairly detailed description here. None of this, 'he was tall and good looking' crap. At least a paragraph on this. It should include physical appearance as well as what your character can usually be seen wearing.
Personality
What is your character like? I don't want any one-liners here, either. 'She is nice' isn't going to cut it. No one is perfectly happy all the time. What makes them tick? Stuff like that.
History
This is optional, but if you have this, you are much more likely to get your audition accepted. It can be as detailed or as vague as you want (details are loved, people) and can include whatever. You want to tell me why Joey's afraid of Pidgeys? Go right ahead. I love reading stories.
